LiveScan Status Report

To maintain confidentiality, your identifier is the first initial of your first name, plus the first three letters of your last name plus the first four digits of date you were fingerprinted for the LiveScan. 
Example: George Smith LiveScan Date – 05/15/2012.  Identifier = GSMI0515.
